How to Complete the Proof of Service & Find Your Court Address

The Proof of Service section certifies that you sent a copy to the government. Here is exactly what to fill in:

① Your Name
Write your full name on the line marked (Name) — same as at the top of the form.
② Date
Write the date you are mailing the form on the line marked (date). This must be within 5 working days of your address change.
③ ICE OPLA Office Address
Write the address of the ICE Office of the Principal Legal Advisor (OPLA) for your Immigration Court. Use the official court locator below to find it.
④ Mailing
Mail the original to your Immigration Court (using the mailing panel on Page 2 of the form) and a separate copy to the ICE OPLA office address you wrote above.
